Since you didn't say what your stradegy is, or what you have already tried I'll have a few questions and will post a few thoughts.

1. Have you tried crating?

2. Have you used a key word to get pup to go...like potty, outside, etc?

3. Do you take pup out after any/all activity? Play, eat, after sleep, etc?

4. Have you considered a doggy door since pup goes to the door?

It sounds like he has the idea of going out, but you are not catching him going to the door. Remember many pups, even at this age, can't hold it very long, so you have to always keep an eye out for subtle signs. Basically he is doing what he is supposed to, but you are missing the signs he is giving to go out.

When you do take him out you need to remember to give him the word you have chosen, and if he does not go, then go back in. No play, just business. Repeat this until he understands the word. This will get to a point that he will respond to it and go. Then you can move onto other things like play. Until that time, he needs to know you are there to conduct business and nothing else.

If you have more specific questions, please post.

Just an idea, but you might want to try to only give him very little ammounts of water between feedings.  And does he poop every time you take him out after eatting?  If not, try to stay out until he has pooped.  It's just to let him know he can't go in until he's finished "business".  I've noticed that when my pup is lying on the couch then gets off (not to retrieve a toy of his) he generally has to use the bathroom.  Maybe you're missing some sort of signal.  XOXO :)

It may have been the puppy pads that have him confused.  Since he was trained to go on the pads in the house when he was younger, he probably thinks that he's supposed to go in the house.
Whenever he makes a move towards the door, let him out.  He should eventually learn that when he goes to the door, he goes outside.  When you take him outside, do not let him back in the house until he does his business.  When he does, imediatly take him back inside and give him his treat.
Other than that, just pay super close attention to what he's doing and what he's sniffing.  I'd say space out the potty times a bit more.  That may teach him to hold it in.  If he's going out every half hour, he probably doesn't have to pee each time.  Try every hour or so instead.

I agree, puppy pads are confusing for dogs, it teaches them its okay to go in the house and then when you try to teach them otherwise they become very confused. They key for house training dogs for me has always been to keep them outside until they do go (and puppies need to go a lot!). Get them to run around outside, this usually kick starts the "urge to go". Also, don't forget to take up the food and water dishes a couple hours before bedtime.

Just an idea...if he still likes the puppy pads, put a pad outside for a few days. This worked for a dog I had several years ago! Hope that helps!!
